Self Certification for illness less than 7 Days

If you have been unwell for 7 days or less, you don’t need to see a clinician. You can complete a Self-Certification form yourself. However some employers insist on a Doctor’s note regardless. A Doctors note under these circumstances may involve a fee.

Many employers have their own self-certification forms. If your employer doesn’t have its own form, you can complete a Self Certification Form. Please fill it in and give to your employer. You do not need to visit your GP surgery for this.

Fit Note for illness more than 7 Days

If you have been unwell for more than 7 days, you will need a medical certificate. These certificates are called ‘Statement of Fitness for Work’ Certificates (Fit Note), and are sometimes also called a sick note.

Please complete an accurx patient triage to request a certificate from the surgery. Please note that we cannot post-date a medical certificate, we can only provide them from the date they are requested, or we can back-date them in certain circumstances.

A clinician may contact you or arrange for an appointment to discuss your condition further, before a certificate is issued.
